Kindergarten registration coming up
WESTFALL The Delaware Valley Elementary School is currently preparing to register eligible students for next year’s kindergarten program. Students must be five years of age on or before Oct. 1, of this year and reside in the Delaware Valley Elementary School area in order to register. Parents who wish to have their child attend next year’s kindergarten and have not received any previous correspondence from the school district must call Robin Adelfio, Delaware Valley Elementary School Secretary, at 570-296-1823 between 9 to - noon and 1 to 3 p.m.. It is important if you wish to have your child complete the registration process scheduled for March that you contact Adelfio as soon as possible. She will need the following information from you when you call: your child’s legal name (last, first, middle), social security number, sex, date of birth, city/state of birth, natural/adoptive father’s first and last name, natural/adoptive mother’s first and last name, mailing address and telephone number. Kindergarten packets will be mailed to parents during the week of Feb. 4. Complete the forms in the packet and return them to the Delaware Valley Elementary School Office along with your child’s original birth certificate, immunization records, social security card, and two proofs of residency including one photo ID (eg. driver’s license, landlord affidavit, deed, mortgage statement, tax bill) during the two weeks prior to your child’s kindergarten registration date. Kindergarten registration will be held on March 13. If the proper documentation has not already been copied or is not in hand at the time of registration, your child will not be registered. Children entering kindergarten for the 2008-2009 school year must have the following vaccinations: Four doses of tetanus and diptheria (fourth dose must be given on or after the fourth birthday or if not fifth is necessary) Three doses of polio Two doses of MMR Three doses of hepatitis B One dose varicella
